Information Submission Interest Form

The Illinois Accountability Commission (IAC) is collecting information related to potential misconduct by federal immigration law enforcement officers during Operation Midway Blitz through the end of 2025 (September through December 2025).

If you would like to submit information to the IAC, please review the following frequently asked questions and complete the interest form at the bottom of this page. The interest form requires you to provide a phone number and email. The form does not ask for detailed information about possible federal immigration law enforcement misconduct. After you submit the form, someone will contact you about next steps and will discuss the best way to submit detailed information to the IAC.

Will the information I share be kept confidential?

  • We cannot promise confidentiality.  Any information you submit or through this process to the IAC may be subject to relevant records disclosure requirements such as the Illinois Freedom of Information Act. Access the State of Illinois Privacy Notice to learn more at illinois.gov/about/privacy.

What kind of information should I share with the IAC?

The IAC is collecting information about misconduct that falls within its mandate, meaning that the misconduct:

  • was perpetrated by federal immigration law enforcement agents, such as agents who work for Immigration and Customs Enforcement (ICE) or Customs and Border Protection (CBP); and
  • occurred in Illinois during Operation Midway Blitz from September 2025 through the end of 2025.
